The final dissemination conference of Project INCA, partially funded through the European Union’s CIP Programme, will take place on Friday, 20 May 2016. This one-day session is free to attend for anybody working or interested in the field of integrated care and related areas and is open to all.

Care delivery happens in a very unbalanced way, some primary care resources are under-utilised (local clinics, nurses) and some acute resources (emergencies, hospital admissions) are over-used. This impacts health organisations efficiency, especially when companies and public sector organisations are attempting to optimise how health resources are utilised.

Average patient satisfaction levels may be low as the general feeling is that professionals are “improvising” while the situation evolves (for example, there is no plan at all on what do or at least no real-time tracking on how plans are being met or not until the patient returns for a subsequent visit).

INCA offers an innovative solution to manage all the challenges healthcare systems need to tackle with a very pragmatic approach to deliver integrated care.
